Neurocirculatory Scalenus Anticus Syndrome in the Presence of Developmental Defects of the First Rib *
نویسنده
چکیده
In years past, a rather voluminous literature has accumulated dealing with certain neurologic and circulatory deficits that occur in the upper extremities. Cervical rib syndrome, scalenus anticus syndrome, Naffziger syndrome, and scalenus neurocirculatory compression syndrome are among the names applied to the variable symptom complex found present in a small percentage of individuals with abnormal morphologic developments about the region of the thoracic operculum. Because so few of those with developmental defects in that region have symptoms attributable to them, and these usually develop their symptoms relatively late in life, various hypotheses have been suggested as the mechanisms for production of the symptoms. Among these are: 1. Alteration in the relative positions of the subclavian structures and the first dorsal rib due to the gradually increasing dependency of the shoulder girdle as one grows older; with a resultant angulation of the vessel over the rib and a pressure of the rib upward against the brachial plexus.7 2. An abnormal development of the first rib or a cervical rib causing pressure upon the plexus.3 3. Variations in the constitution of the brachial plexus whereby the second dorsal segment takes a greater than normal part in the formation of the plexus.4 4. Theories taking into account the scalenus anticus muscle as a factor in compressing the nerves and artery against the abnormal rib or the normal first rib in cases where the relative positions of the rib and shoulder girdle are altered.3 " 6 While some would attribute the circulatory manifestations to direct compression of the subclavian artery, others believe them due to interference with the vascular innervation.2

Commentary: Comments on Thoracic Outlet Syndrome
74 Thoracic outlet syndrome (TOS) is a comprehensive term used to describe symptomatology due to compression of neurovascular bundle in the thoracic outlet region of upper extremity. The term TOS was used for the first time in the literature by Rob and Standeven.[1] It had also been termed costoclavicular syndrome, cervical rib syndrome, scalenus anticus syndrome, subclavius tendon syndrome, or...
متن کاملAn Instance of a Postesophageal Left Subclavian Artery, an Ununited Right Oviduct, and the Absence of the Right Kidney in the Same Individual
The following unusual variations were observed while making a routine dissection of a newborn white child. The left subclavian artery arises from the right subclavian on the right side of the neck. It then curves upward and to the left and passes dorsally, in succession, the right internal jugular vein, common carotid artery, vagus, cervical sympathetic and inferior laryngeal nerves, and the sc...
متن کاملThe role of the first rib in the scalenus anterior syndrome.
The scalenus anterior muscle was first believed by Murphy (1905) to play a part in the production of symptoms by a cervical rib and since then its importance in a proportion of these cases has been recognised. Adson and Coffey (1927) claimed that division of the muscle, without resection of the cervical rib, was sufficient to relieve symptoms. Naffziger (quoted by Ochsner, Gage and De Bakey 193...
متن کاملHypertrophic non-union of the first rib causing thoracic outlet syndrome: a case report.
We experienced a rare case of thoracic outlet syndrome caused by hypertrophic nonunion of the first rib. A diagnosis was made mainly upon provocative tests and imaging studies. Pain and tingling could be reproduced and the radial pulse obliterated by the hyperabduction test. Abundant callus formation on the posterior aspect of the first rib with fracture line was visible on plain radiograph. Tw...
متن کاملAn Anomalous Infrahyoid Muscle
During the dissection of a cadaver an anomalous infrahyoid muscle was discovered. This was unlike those variations of the infrahyoid muscles described previously.`~The anomalous muscle is classified as a cleidohyoid muscle because it arose from the clavicle and inserted on the hyoid bone. The previous reports2"'5 of anomalous cleidohyoid-type muscles are general descriptions which lack diagrams...
